THE BILLING PROCESS
DermTech will first submit your claim for the PLA test directly to your insurance company after reporting your test result to your doctor.
EXPLANATION OF BENEFITS
After processing your claim, your insurance company will send you an Explanation of Benefits (EOB). The EOB is not a bill. You should review and contact us at 1-866-450-4223 with any questions.
If your insurance company does not cover all or a portion of the cost of the test, we will appeal the decision on your behalf. We may require a patient consent form, which we will forward to you.

PATIENT BILLING INFORMATION
As a courtesy, DermTech will submit your claim to your primary insurance carrier for the PLA laboratory test for detection of melanoma.
After processing your claim, your insurance company will send you an Explanation of Benefits (EOB). The EOB is not a bill. Please review this document and contact DermTech’s Patient Services Team at 866.450.4223 x 155 with any questions or concerns.

Depending on the arrangement your health insurance plan has with us, you may or may not receive a bill directly from DermTech. We do our best to obtain reimbursement from insurers without contacting you.
- Financial Assistance Program: DermTech offers financial assistance to reduce out-of-pocket costs for qualified underinsured patients in the US. Examples of patients who are considered underinsured include:
- Patients with a high deductible or out-of-pocket expenses
- Patients with an insurance policy that does not cover DermTech laboratory services

Why did I receive a bill from DermTech?
Your physician selected DermTech laboratory based on our unique test for melanoma using the adhesive skin collection kit. Your bill from DermTech usually pertains to a deductible or copayment for this service.
Why did I receive a bill from a laboratory?
If you or your insurance company received a bill from DermTech, it is because we provided pathology laboratory services to your physician on your behalf.
What do I do if I receive a check from my insurance provider?
If your insurance provider forwards you or the policy holder a check for DermTech services: Please sign the back of the check, write “Payable to DermTech” and forward the check to DermTech. Please include a copy of your EOB along with your check. The money paid by your insurance provider is due to DermTech upon receipt.
